We are excited to partner up with one of Sydney's most successful Rehabilitation providers in the search for an experienced Case Manager!
They are looking for a passionate, empathetic individual who is reliable and motivated. As the cases can be quite complex, they are seeking an individual who is passionate about medical management scenarios which may include brain and spinal injuries, mental health and comorbidities.
About the Role:
- Liaise with key stakeholders, managing access and seeking funding for services under relevant legislative frameworks
- Carry out discipline specific services as required
- Regular case management of complex physical and/or mental health needs
- Working with people who may face multiple and/or complex barriers to life and work
- Requires excellent verbal and written communication
- Excellent KPI structure reflecting excellent injury management services
About You:
- Relevant Qualifications (Occupational Therapist, Exercise Physiologist, Psychologist, Rehab Counsellor, Registered Nurse, Social Worker)
- Professional Registration with the relevant body (If applicable)
- Min 3 years experience within complex case management (Desirable)
- Have a current drivers license and insured vehicle
- Current WWCC and Police clearance
- Experience in Occupational Rehabilitation and challenging situations (Desirable)
- Fully vaccinated against COVID
